Changes to Disabled Veteran License Plate, Form VTR-615

We are informing you of a recent design change to the Application for Military Disabled Veteran License Plates and Parking Placards, Form VTR-615. This form is used by disabled veterans to apply for their military specialty license plates through their local county tax assessor-collectors office.
The form content was expanded to meet requirements set by the 84th Legislature. Changes to the form include adding 22 new Disabled Veteran license plate options, allowing a military identification card as proof of eligibility, and dates of service for the new theater of battle emblems/medals.
The form has also been changed from a single document to a two-part document in order to accommodate the additional content.
The Application for Military Disabled Veteran License Plates and Parking Placards, Form VTR-615, is a two-page application to be completed by the applicant and submitted to the county tax office.
The Instructions for Application for Military Disabled Veteran License Plates and Parking Placards, Form VTR-615-I, is a new, two-page instruction form.
Both forms are downloadable from the Texas Department of Motor Vehicles (TxDMV) website. You may find it useful to keep a copy of the instruction form on-hand in your office(s) for staff and customers to review.
No changes were made to Disabled Veteran license plate fees or the requirement to submit these applications and supporting documents to the Special Plates Unit for archiving. Please do not include the Form VTR-615-I with the applications.
