Updated Supply Order Form (VTR-240)

The updated Supply Request (Form VTR-240) is attached for your immediate use.

Please discontinue use of previous versions of the form.

Any order submitted on a version of a form prior to the 12/2021 revision will be rejected for resubmission before it can be processed. Please continue submitting the Supply Request (Form VTR-240) to your local Texas Department of Motor Vehicles (TxDMV) Regional Service Center.

As a reminder, please continue to check the “ORDER BY” formats, and make sure that items are ordered in the proper increments. If the item description instructs you to order by box, package, ream, etc., verify you are ordering by that increment.  The “YOU WILL GET” column will show the total number of items to be shipped with the processed order.  Review orders to verify that the total in the “YOU WILL GET” column is correct before submitting the Supply Request.
